Admissions Manager-Hospice Job in Stuart 34997, Florida US

DEFINITION: 

Responsible to the Director of Admissions and Access for assigned functions related to providing information about services to potential Hospice patients and coordinating the admission process.

RESPONSIBILITIES: 
The Admissions Manager works with the Admissions Coordinators and Admissions Nurses to coordinate  admissions to the hospice program.  The Admissions Manager also triages and prioritizes the referrals so that all admissions are processed in a timely manner with appropriate follow-up. They also assist with continuity of care and discharge planning in hospitals, especially those outside our service area. Once patient is admitted they ensure smooth transition of patient to the Interdisciplinary team.

QUALIFICATIONS:

• RN with a minimum 3 years experience including 1 year in an acute care setting.  Hospice experience preferred but not required.  Experience should include the following:  Case Management or Admissions experience in acute care or long term care setting.   

• Demonstrated competency through experience, in coordination of activities, referrals and admissions.  

     
• Demonstrated competency to relate effectively with patients, staff physicians, community agencies and institutions.
     
      
• Current Florida driver’s license and automobile insurance.

FUNCTIONS:
• Plan for admissions to Hospice.  

• Assume initial responsibility for assessing patient/family needs, obtaining necessary documentation of terminal status and consulting with attending physician, other hospice staff members as well as the admission staff.

• Oversee admission of patients, disseminate information and complete related paperwork in accordance with Hospice policies and procedures.

• Communicate on a continual basis with the Director of Admissions with any unusual or potentially problematic patient/family issues, plan for admissions and activities for the day.

•  Facilitate transition of patient/family already in care to hospice house staff at appropriate time.

•  Screen and process inquiries for admission to Hospice care

•  Determine patient’s appropriateness for Hospice services consistent with Hospice service policies and admission criteria in conjunction with patient’s physician and medical director.

• Make or delegate the telephone contact and/or assessment visits with potential hospice   patients to provide information on Hospice services and/or assess patient’s status.

•  Assist with implementation and coordination of services required for discharge from hospitals into Hospice’s program of care.

• Share in the admission process when necessary and weekend/holiday coverage during vacations or emergencies.

• Responsibility to ensure phone coverage for the Admissions Department, assignment of planned admissions and scheduled breaks for the staff to ensure that activity is staggered rather than clustered and that someone is always available to cover the phone.

•  Participate in agency and community programs as requested to promote professional growth and understanding of hospice care.    

•  Demonstrate familiarity with policies of the agency and rules of regulations of State and Federal bodies which aid in determining policies.   

•  Assist Director of Admissions and Access  with day to day operations as needed.

•  Possesses excellent critical thinking skills.

•  Accepts other assignments as appropriate.
