POSITION SUMMARY:

The Nurse Practitioner (NP) acts as part of the clinical services team and may be providing assessment visits in assisted living settings, nursing homes, skilled nursing facilities (SNF) and home settings depending on the nature of the assignment; or providing direct care delivery to members in skilled nursing facilities as a member of the medical management team.  The responsibilities may include:  history review, health education, physical exam and diagnosis confirmation in the home care, SNF and assisted living settings.  Diagnosis and treatment according to current protocols for patient encounters per day will be provided by the NP in Medical Management Services.  Provides referrals and participates in follow-up.  Consults with the Manager to ensure compliance with guidelines.  Participates in Risk and Quality Management programs, clinical meetings and other meetings as required.
