NURSE PRACTITIONER OR PHYSICIAN ASSISTANT

 

 

Position Summary:

 

Provides home-based primary care, care management, and care coordination for a specified panel of adults and elders with disability and chronic, complex needs (approximately 40-50 for 1.0 FTE) in need of intensive care management.

 

Essential Duties and Responsibilities:

 

·           Coordinates care for patients to ensure continuous, comprehensive care 

·           Functions as a primary care nurse practitioner or physician assistant for a specified panel of patients

·            Maintains appropriate written and oral communication

·            Conducts educational and training activities that promote appropriate and effective patient care

·            Participates in quality assessment and quality improvement within the scope of responsibilities as appropriate

·           Actively participates in the evaluation of own performance

·            Seeks to maintain a constructive work environment and maintains effective communication with other employees and managers

·            Makes decisions within the scope of the job, reflecting knowledge of facts and care for patients and employees

·            Maintains confidentiality of client and employee information

·            Communicates a positive image as a representative of BCMG


 

Requirements:

 

Education / Licensure

Master’s degree in Nursing

ANA certification as an adult, geriatric, or family nurse practitioner

Current Mass license as a Registered Nurse/NP

                                                or

Graduate of an Accredited Physician Assistant Program

Current Massachusetts Licensure/eligibility for prescriptive privileges.

 

Experience

 

·            Three years relevant clinical, homecare, and case management experience desired.

Physical Abilities

 

·          Ability to enter, exit, and drive a car to a variety of locations within Eastern Massachusetts on a daily basis

·          Ability to climb stairs

·          Visual/hearing ability sufficient to comprehend written/verbal communication and perform physical exams and assessments of patients.
