The Elder Service Plan is a comprehensive program designed for frail elders who are nursing home eligible but wish to remain at home. The program provides general primary care, rehab, and wellness services at an Adult Day Center and at home. It also uses a team approach to coordinate all health, medical and social services needed to maintain the frail elder's independence. It is currently available in Somerville, Cambridge, Malden, Allston, Arlington, Brighton, Chelsea, Everett, and Medford. The Elder Service Plan of Cambridge Health Alliance operates under a contract with the Massachusetts Departments of Medical Assistance and the Centers of Medicare and Medicaid Services. The Nursing Assistant position with Elder Service Plan will be responsible for the following: 1. Responsible for personal care, Medication reminders, meal preparation and light housekeeping 2. Excellent customer relations skills with patients, visitors, physicians, and co-workers. 3. Must organize daily work, prioritize, and make decisions concerning care to meet the needs of individual and groups of patients. 4. Must adhere to and administer treatment according to Cambridge Health Alliance's policies and procedures. 1. Certified as a Nursing assistant in MA 2. Two years experience with frail elders. 3. Current Basic Life Support per American Heart Association Guidelines 4. Computer skills 5. Ability to work independently 6.
