Position Name: Palliative Services Manager
Position Summary:
· To assess applicants needs and suitability for care at Perram House
· To facilitate program development implementation and evaluation
· To manage and lead the clinical care team.
Specific Responsibilities
Management
· Manage staff with overall responsibility for the hiring, performance review of all members of the clinical care team.
· Schedule personnel to ensure proper and appropriate staff coverage
· Review and approve reported time cards
· Promote a culture in which staff foster ad further evidence based best practice and interdisciplinary team approach
· Provide support to the Clinical Care Coordinator in staff meetings to discuss progressive disciplinary measures with staff.
· Lead and participates in staff meetings
· Be a clinical resource for the team
· Bereavement support for family members
Program Supervision
· Ensure that programs comply with LHIN(s), MOHLTC and CCAC requirements as well as palliative care standards / policies and procedures.
· Ensure that professional standards are maintained and legislative requirements are adhered to
Education
· Remain current with trends, issues and regulations associate with the delivery of care services.
· Coordinate and facilitate research, planning, implementation and evaluation of best practices
· Coach mentor and orient the interdisciplinary team and/or facilitate such processes
· Continuously engage staff and volunteers in enhancing palliative service options and interdisciplinary team opportunities.
· Provide clinical education and support re:
o Proper implementation of infection control policies / procedures and
o The proper use of protective equipment
· Liaise with the infection control network re: infection control issues in the community.
· Plan regular training and education for staff members on topics relevant to their responsibilities
· Volunteer education
Administration
· Establish and maintain collaborative relationships with external stakeholders to promote and provide clinical services
· Monitor the supply of personal protective equipment available for staff and volunteers as part of pandemic planning
· Facilitate resident transition planning (new admissions or discharges to other locations)
· Serve as infection control champion for the house.
· Incident reports
o Review all incident reports
o Lead monthly team reviews to promote learning
o Creation and implementation of changes to clinical programs, specific resident care, policies, etc in response to the findings of incident reports.
Program development
· Leads and participates in strategic and related service planning
· Particpates and develops an annual operating plan
· Develops, and reviews policies, procedures and guidelines that contribute to improved client care and care outcomes.
Qualifications
· Must be registered with the College of Nurses of Ontario
· Must have a current CPR cerfitication
· Minimum of 2 years of current Palliative / end of Life care
· CNA Certification in Hospice palliative care Nursing (or willing to obtain)
