Admixture Pharmacist

Oversees participates in the preparation of sterile products coordinates the efficient operation of IV admixture services, oncology services or allergen services.
Essential Functions:
- Prepares, procures, packages, stores distributes sterile products supplies for oncology, clinic, Home IV antigen patients w/in established procedures legal requirements. These therapies include, but are not limited to: antibiotics, antineoplastics, total parenteral nutrition, small large volume parenterals, pain medications, sterile ophthalmic preparations allergen extract preparations, where applicable. Document responsibility by signing prescriptions, batch sheets other forms.
- Reviews physician orders provides drug information pharmacokinetic advice when needed. Functions in the chemotherapy, I.V. drugs antigen distribution system for home, infusion center, SNF or medical center clinic use.
- Monitors drug therapy, lab tests, confers w/ medical staff providers suggests changes in drug therapy, drug utilization or therapy monitoring methods as appropriate. Documents all clinical interventions quality problems using the approved methods.
- Explains procedures of the I.V. program to a patient or family member counsels on their medications. Distributes drugs, flushing delivery system educational materials to the patient or family member.
- Orders maintains drug, supply, equipment inventories for the delivery of the admixture antigen service. Maintains work area to ensure storage work areas are clean organized for a safe work environment. Resolves drug, supply equipment problems as needed.
- Demonstrates professional competency in accordance w/ current sterile compounding standards participates in ACPE accredited continuing education programs. Assists in the training education of pharmacy technicians, students residents.
- Provides direction to the pharmacy staff in resolving professional /or clinical problems, trains employees in new procedures techniques, monitors department staff for errors adherence to the established department procedures.
- Demonstrates proficiency in operating automated systems for prescription order entry, patient profile review or retrieval of clinical or statistical data. Maintains proper records related to medication/supply charts, controlled prescription record keeping other legal department records procedures. Demonstrated proficiency w/ the pumps used in oncology Home Infusion, where applicable.
- Participates in departmental quality improvement initiatives contributes to team or departmental projects directed toward the achievement of decreased costs, improved service efficiency patient outcomes.
- Assures compliance to all regulatory rules regulations KP's policies procedures to include but not limited to:
- Drug Enforcement Administration (DEA)
- State Boards of Pharmacy
- HIPAA
- Principles of Responsibility
- Proper use of KP personal computers applications
- Provides after hours pager coverage as required.
- Performs special duties projects as required.

Schedule Details:
- This position has a rotating schedule to include working at nights, weekends and holidays.
- This is an ON-CALL position for Maryland and DC IV admixture pharmacist to staff our Maryland and DC Oncology / Infusion pharmacies when needed.
- This job does not come with any committed hours.
- It is a PRN on-call position with expectation to staff at multiple pharmacies within the region when necessary in Maryland and the District of Columbia.

Basic Qualifications:
- 1 year of home infusion or hospital admixture experience required.
- BS in pharmacy or PharmD required.
- Licensed to practice pharmacy in the assigned jurisdiction required.
Preferred Qualifications:
- Board certification in specialty or home infusion certification preferred.
- Infusion and Oncology pharmacy experience.
