Alternate Medical Director
Saginaw, MI
MAJOR JOB FUNCTIONS:
- Assumes duties of the LMD in the event the LMD is unavailable.
- Assumes responsibility for the medical component of the hospice patient's care program.
- Attends all Care Team Meetings as Physician member of the Interdisciplinary Team.
- Reviews, revises and documents the Plan of Care no less than every 15 days.
- Certifies patient's medical eligibility for the hospice services in accordance with the Hospice Program Policy.
- Recertifies eligibility for subsequent election periods in accordance with the Hospice Program Policy.
- Coordinates with attending physicians to provide care when attending physicians are unable to retain responsibility for patient care.
- Collaborates with attending physicians regarding hospice admissions and pain and symptom management.
- Acts as medical liaison with physicians in the community.
- Acts as medical resource for the Interdisciplinary Team.
- Available to hospice 24 hours a day, seven days a week.
- As a member of the Quality Assurance Performance Improvement Committee (QAPI), reviews and analyzes QAPI projects and information.
REQUIREMENTS:
- Licensed doctor of medicine or osteopathy.
- Experience and interest in the psychosocial and medical aspects of hospice care.
