Asthma Educator

Contractual Position: Asthma Educator

 

Duties: Coordinates resources with the WCHAP team and provides asthma education and case management services for at-risk Medicaid children with asthma.

 

1. Develops and implements asthma educational services for Medicaid children with asthma involved with the Wayne County Children’s Medical Home Project (WCHAP).

 

2. Performs Asthma Case Management focusing on high needs Medicaid children with asthma:

a. Identifies Medicaid children with asthma

b. Stratifies high-risk patients

c. Provides asthma education/case management services to better control exacerbations, decrease utilization and improve school attendance

 

3. Assesses, plans, implements, evaluates and documents asthma educational interventions, including home visits, with case-managed children and their families.

 

4. Researches and implements new asthma education and case management strategies.

 

5. Establishes and nurtures professional relationships with schools, physician’s and office staff to develop and enhance effective asthma management across the continuum.

 

6. Facilitates effective communication between family/physician and family/school.

 

7. Initiates interdisciplinary referral process.

 

8. Interacts with WCHAP project team, including participation in regular meetings.

 

9. Tracks interventions and case management outcomes through ongoing data collection (i.e., Quality of Life Questionnaire, pre/post tests, chart reviews, AN database).

 

10. Develops and implements protocols for group visits and/or one-on-one education in a variety of settings.

 

11. Meets regularly with managers of all pilot practices to discuss issues and improve processes.

 

12. Acquires and maintains current knowledge in asthma management.

 

13. Accountable for own professional development

 

14. Performs other duties as assigned

Asthma Educator

Qualifications:

 

Education and/or Experience:

 

Bachelors of Nursing* required.

1.      Three years experience in program development; clinical and health education and demonstrated success in the area of asthma management, or related respiratory area expertise. **

2.      A working knowledge and experience with asthma management techniques as described and defined with the National Guidelines for Diagnosis and Management of Asthma (National Institutes of Health, National Asthma Education and Prevention Program, August 2007) is required.

3.      2 years home health or school based experience preferred

4.      Possess the knowledge, skills and experience necessary to perform effective case management.

5.      Experience in a team environment.

6.      Current license to practice nursing in the State of Michigan.

7.      Valid Michigan driver’s license and automobile insurance coverage.

8.      Knowledge/experience of complex care of pediatric patients.

9.      Strong working knowledge of families within the community systems, and/or the Michigan Medicaid system.

10.  High level of analytical ability; reviewing and identifying issues within reports, as well as analyses of child/family issues across the continuum.

11.  Excellent communication and interpersonal skills needed to provide effective collaboration with patients, family, health care team and any other external entity necessary across the continuum of care.

12.  Computer skills include a working knowledge of Word, Excel, PowerPoint, and relational database applications.

 

 

Relationships: Work under the direction of the Nurse Coordinator.

 

Compensation: $25-30per hour.  24-32 hours per week

 

 

 

*Associates Degree in Nursing with three years’ experience can apply

**Applicants not certified as an asthma educator, but with complimentary experience must achieve Asthma Certification within six months of hire
