Band 6 CHC Nurse Assessors

Our client in South Birmingham is currently looking for Band 6 CHC Nurse Assessors. The positions required will form part of the Continuing Healthcare division for an initial period of 3 months with the possibility of extension. These roles are all offered on a full time basis, 37.5 hours per week, Monday- Friday.

The successful candidates will be required to carry out Continuing Health Care Assessments for patients in their own homes, on their discharge from hospital, as well as in nursing homes. The role involves dealing with individuals with varying health care needs, their carers and families, a range of Providers and Health and Social Care Providers with the aim of determining their eligibility to have their package of care funded by the NHS.

The main tasks and duties involved in this role include:-
• Producing comprehensive Needs Portrayal Documents for patients whose relatives or carers have requested an assessment of their eligibility for continuing NHS health care
• Selecting and utilising relevant evidence from GP records, hospital records, social care assessments and community health service records.
• Ensure that the claims made by enquirers (the relatives or carers of individuals seeking to have their care funded by the NHS) are validated so as to ensure the appropriateness of a retrospective assessment.
• Remain in contact with enquirers, updating them of the progress of their enquiry.
• Present and defend the Needs Portrayal Documents at an Independent Reassessment Panel Hearing
• Actively contribute towards Team Meetings, Clinical Supervision sessions, training courses and audits
• Follow policies and procedure established to ensure that each enquiry is treated consistently.

The ideal candidate will have:-
• Previous experience as a Continuing Healthcare Nurse Assessor (essential)
• Knowledge of the Department of Health National Framework for NHS Continuing Healthcare.
• Knowledge and experience of patients with multiple illnesses or conditions and the impact this has on their health and well-being.
• Experience of making assessments against identified standards
• Knowledge and experience of risk assessments, clinical governance, policy and audits
• The ability to build strong relationships with clients
