BFT - NURSE PRACTITIONER FOR LONG TERM TEMP ASSIGNMENT- JUNE THROUGH SEPT
General Overview Bulfinch Temporary Service is currently hiring a Nurse Practitioner for the MGH Diabetes Center. The assignment is from June through September, 40 hours a week. The hours are Monday through Friday from 9am-5:30pm. The Nurse Practitioner is an advanced practice nurse who will be responsible for providing patient care and education for Diabetes and Primary Care patients in collaboration with our physicians. This includes health assessments, extensive diabetes education, physical exam, telephone triage, and referrals. The nurse will also provide evaluation of diabetes self-care, education and management to patients with diabetes referred to the practice. The Nurse Practitioner should be experienced and familiar with specialized Diabetes and Primary Care and be comfortable making adjustments for insulin, insulin pumps and making primary care decisions. Responsibilities 1. The Nurse Practitioner of the MGH Diabetes and Internal Medicine Center functions in an expanded role, responsible for daily management of patients with diabetic problems and/or other intercurrent illnesses. The NP is responsible for assessment and management of the patients, including diagnostic and therapeutic interventions in collaboration with the attending physician. The nurse practitioner will provide extensive education as well as patient and family psychosocial support. 2. Perform new patient evaluations obtaining: ? Clear Chief Complaint(s) and current issues ? Detail and accurate current medication list and drug allergies ? Detailed Past Medical Hx ? Pertinent Family Hx ? Social Hx and Habits ? Review of systems ? Vitals; Ht, Wt, BP, Pulse prior to MD consultation. 3. Utilizes advanced nursing knowledge to identify, prevent or solve complex and/or recurring patient care problems. 4. Participates with other nursing staff in formal Diabetes Education Program as an instructor, and coordinates ongoing classes and patient enrollment. 5. Provides direct patient education on disease management strategies, therapeutic goals, and disease prevention. 6. Works with the other nurses to process prescription refills, answer medication questions, triage emergent patient phone calls, and process paperwork as necessary for timely medical care. 7. Communicates with physicians, and other necessary clinicians, via verbal and written documentation to facilitate a comprehensive plan of care and coordinate resources. Documents patient visits and patient correspondence through hospital computerized medical record system. Qualifications 1. Registered professional with additional specialized formal education in physical assessment, management of the disease entities of the adult including educational and psychosocial needs. 2. Qualified to assess and recognize changes in the physical condition of the adult patient, in particular, he/she with diabetes, notifying the supervising physician when necessary. The nurse practitioner functions interdependently with the supervising physician or designee, in order to provide optimum health care to patients. 3. Masters degree in Nursing. 4. A record of license as an Advanced Practice Registered Nurse from the state of Massachusetts and a copy of the nurse practitioners certification will be obtained.
