Allina Commons is the Minneapolis-based headquarters of Allina Hospitals Clinics, a not-for-profit health care organization serving patients and communities. Approximately 1,600 corporate and administrative service employees work at offices within Midtown Exchange, site of the historic Sears retail complex at Chicago Avenue and E. Lake Street. Responsibilities:
- The Care Manager works to establish collaborative processes that promote quality and cost effective care that optimizes / enhances the physical and psychological health of patient across the health care continuum.
- The Care Manager is responsible for making decisions that impact health care outcomes, resource allocation and customer feedback.
- Provides care management to patients identified through referral to specialty care; manages projects and participates in committees as appropriate; interacts with and supports the specialty care provider and interdisciplinary care team.
Qualifications:
Education:
- Bachelors degree in nursing or other health care related field preferred.
Experience:
- Minimum of 5 years of clinical experience required with 2-3 years of experience in speciality area; previous care management or homecare experience preferred.
Skills:
- Demonstrated ability to work and collaborate with primary care providers; demonstrates sound knowledge bases and actions in the decision making process for designated patient populations; ability to analyze data and interpret reports; ability to asses trends, ability to take in and interpret large quantities of complex information. Demonstrates a high level of verbal communication skills and has ability effectively communicate with staff, providers, patients, families and outside agencies.
- Position can be highly stressful -ability to manage stress effectively and ability to set priorities.
RN hires with less than 1 year of RN experience must be a graduate of an accredited school of nursing (American Association of Colleges of Nursing or The National League for Nursing) which is inclusive of a clinical rotation in an acute care facility.
Pay,Benefits, Work Schedule:
- Full Time (40 hours).
