Emergency Department Care Manager

Mercy Suburban Hospital, an operating unit within the Mercy Health System of Southeastern Pennsylvania and located in East Norriton, PA, has an immediate opportunity available in the Emergency Department for a Care Manager. This is a full-time position on the day shift, reporting to the Director of Care Coordination.

Primary Responsibilities include:
?Assisting physicians and the interdisciplinary team in facilitating the entry of patients into the appropriate level of care utilizing Interqual criteria
?Reducing unnecessary admissions to the acute care hospital
?Assigning the correct levels of care to ED patients (outpatient observation vs. inpatient admission and recommending options for safe discharge
?Coordinating all non-elective point of entry admissions and facilitating timely throughput of ED patients, ensuring safe delivery of services at the appropriate level of care
?Identifying community and other resources necessary for each individual patient and matching specific needs for continued care
?Identifying services that require authorization and collaborating with payers to obtain the needed authorizations for services
?Ensuring patient has primary care follow up
?Collaborating with the ED healthcare team to determine treatment plans, while observing quality and patient safety parameters, LOS, readmissions, denials and appeals

Candidates will possess the following qualifications:
?Bachelors of Science in Nursing with current PA RN license
?Three years of case management experience including emphasis on discharge planning
?Knowledge of Managed Care, government payers, third party reimbursement, Interqual criteria
?Ability to handle multiple tasks simultaneously, effectively collaborate with healthcare team members, communicate clearly in writing and orally, and document in an electronic medical record completely and accurately
?Knowledge of information systems and MS Office applications; Midas preferred
?Case Management certification from an accredited organization is preferred.
