CARE MANAGER
(GERIATRIC SUPPORT
SERVICES COORDINATOR)
Performs assessments regarding the health and functional
status of seniors; determines appropriateness of institution-
al long-term care; develops community-based care plans;
and arranges for necessary services. Requires a Bachelor's
degree and at least two (2) years professional experience
in the care of elders (60 years ). Must have a valid driver's
license and reliable transportation. 33 hours/week.
$19.93 per hr; competitive benefits.
REGISTERED NURSE
Participate in determining personal care needs and
establishing care plans in accordance with applicable pro-
gram requirements. Maintains collaborative relationships
with community agencies, institutions, social and medical
services for clients. MA RN license and valid drivers license,
and reliable transportation required. 35 hours/week;
$25.31 per hour.
