Care Manager, Social Worker

Position Overview:

 

As part of the Care Management team the Social Worker participates in assessing and evaluating the needs of members, and works with the member and the rest of the team in planning and implementing care that addresses the care needs of the member.  This position is based in the community approximately 60% of the workweek. This work is carried out in support of the mission and goals of Independence Care System.  

 

Key Essential Functions:

 

1.       Assess member’s status and needs, conduct onsite visits, as needed.

2.       Completes Comprehensive Care Plans - develops a care plan tailored to meet member’s needs, issues and desired outcomes.

3.       Collaboration - Participate with other team members in developing and monitoring Member care plans, including participation in care team meetings.

4.       Documentation, Reporting and Compliance – complete documentation, and reports that adhere to timeframes and ICS guidelines.  Document care plan in member’s record, member contacts, and revise written care plan as needed. 

 

Qualifications:

 

1.   Bachelor’s in Social Work (BSW) required.  Master’s Degree in Social Work (MSW) preferred.

2.   Minimum two (2) years work experience in the social or health services fields.

3.   Experience in conducting assessment interviews and documenting.

4.   Highly self-motivated and ability to manage time independently.

5.   Comfortable with extensive travel in Brooklyn, Manhattan, the Bronx, and/or Queens.

6.     Public health, community-based or home care experience preferred.

7.     Geriatric, disability or rehabilitation programs preferred.

8.     Bilingual – (English/Spanish or other language) a plus.
