Case Manager

The Blended Case Manager will provide resource coordination, recovery services, and symptom management using a strengths-based model of case management to 20-25 Adults living with a mental illness. 

Services Include:

The BCM will also provide information and support to families and significant others of the individuals served as well as complete required paperwork, correspondence, and chart documentation. 

Salary: $14.91 / hr

Monetary incentive program.  Travel within service area required.
