Case Manager for United Healthcare Community and State

Responsibilities for Case Managers include:
-Engaging members telephonically to coordinate services, community resources, and treatment needs
-Delivering a holistic approach to coordinated care based on the enrollee needs
-Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file
-Creating, reviewing and revising Plan of Care plans and focusing on disease management
-Functioning as an advocate for members and decreasing unnecessary hospitalizations
-Collaborating with the member's PCP to deliver and coordinate necessary services
-Building relationships with members and their families; assisting them with proper health choices
-Ensuring cases are documented in a timely manner

Positions in this function require education/certification/licensure other than an RN, such as BSW, MSW, Respiratory Therapist, Dietician, Nutritionist, Occupational Therapist, etc. Positions are responsible for providing clinical and medical management services, including case management, health assessments, interventions, and discharge planning. May identify, coordinate or provide appropriate levels of care under the direct supervision of an RN or MD. This function does not include Health Coach, Health Educator and Health Advocate roles that do not require an RN. Those roles are found in the Wellness Coach Management function.
- Generally work is self-directed and not prescribed.
- Works with less structured, more complex issues.
- Serves as a resource to others.
Requirements:
-Bachelor's level degree in Social Work or other related field
-Minimum of 1 year clinical or case management experience
-Minimum of 1 year experience in long-term care, home health, hospice, public health, or assisted living
Additional Assets Preferred:
-Medicare/Medicaid experience
-A background working in geriatric special needs
-A background in behavioral health
-Experience with electronic charting
-Ability to multitask
-Basic level of proficiency in PC-based word processing and database documentation (Word, Excel, Internet, Outlook)
