What you’d do:
You’d facilitate patients’ access to quality, cost-effective health care; develop and monitor individual goals and interventions in collaboration with providers; and report your findings, recommendations, and outcomes.
What you bring:
NOTE: This is a full-time position; it is expected that the work will be performed on a telecommuting basis upon successful completion of initial orientation and training.
You have at least four years of clinical experience and a Bachelor's degree in nursing or at least six years of such experience and an Associate’s degree or diploma in nursing. Current, active, and unrestricted Alaska RN licensure is also required. CCM is a plus.
