The following opportunity exists within our 343-bed Hemet Valley Medical Center in Hemet, CA:
Case Manager
The succesful candidate will under general direction, coordinate a continuum of care for a selected group of patients from admission through discharge; assure that patients and families proceed efficiently through the course of hospitalization; move patients through the hospital system in a cost effective manner resulting in the optimal discharge plan.
Position requirements:
Education
Associate of Science Degree in Nursing, Possession of current California Registered Nurse License
Certifications
- Possession of current Licensed Vocational Nurse license or California Registered Nurse License
- Current BLS certification
Experience
Two (2) years recent acute hospital nursing experience in UM, DC planning, Case Management or equivalent.
