Position Description:
An LPN with LTC case management experience to assist with members and coordinating care through the medical delivery system plus responsibilities in the LTC facilities to support the NP clinical delivery model, i.e. completion of new patient data, entering diagnoses, allergies, advance directives, medications, immunizations, assisting with non-formulary medication requests.
This position will require travel primarily in Maricopa County.
