Case Manager/Discharge Planner
Montrose, Colorado
The Case Manager will coordinate care and services of patients across the continuum of illness, promote effective utilization of health care resources and work collaboratively with the interdisciplinary team to achieve optimal outcomes. This position will perform a variety of activities which may include the use of clinical protocols and variance analysis, appropriateness determinations, protocol monitoring, plan development and patient and family teaching. Timely and appropriate discharges will be accomplished through assessing patients post discharge needs. Therefore the Case Manager must be knowledgeable of community resources, alternate care facilities and funding issues.
Case Manager must be a Registered Nurse with current license in the State of Colorado.
2 to 3 years experience in a clinical area and 2-3 years experience in utilization review and/or case management. Discharge planning experience helpful. Must have strong interpersonal skills, knowledge of the payer industry, resource management and reimbursement are essential.
