Position Description To manage utilization and appropriateness of care to home care patients. Graduate of accredited school of nursing or Physical Therapy; Current Colorado RN/PT license, BSN preferred; CCM desirable. Minimum of 2 years home health experience, 1 ? 2 years supervisory experience and case management experience preferred Voted Home Care Elite for 2011.
