Quality Life Management is seeking an experienced CNA. Quality Life Management (QLM) started in 1997 with a “clear commitment to a standard of excellence in providing a continuum of specialized health care services to residents and their families.” At QLM, our mission is a simple statement that describes a complex passion necessary for us to succeed in our goal. Currently QLM manages ten skilled nursing and three assisted living communities in Colorado. There are approximately 1200 employees working within our communities and due to our recent volunteer recruitment and programming focus, we now have seventy five volunteers supporting the needs of our residents. Our skilled care communities range in size from 40 to 110 beds and we are proud of the unique care models offered within each including an adult drug and alcohol dependency program, dementia and Alzheimer’s specialty care, short-term and extended care for those with mental illness, short-term rehabilitation, long term care, hospice, palliative and complex nursing (including ventilator) care. Our facilities are predominately located within the Denver-metro area; however, we do manage care communities in the Grand Junction and Pueblo communities. Within our ten skilled nursing and three assisted living communities, we provide distinct and unique care models for nearly 1000 individuals including the frail elderly, and the medically complex; additionally, we serve a need-based population including care for the physically and mentally impaired younger adult.

Position Title: Certified Nursing Assistant (CNA) – Full-time evenings 2:30pm-10:30pm including weekends
BASIC FUNCTIONS
The primary responsibility of your job position is to provide each of your assigned residents with routine daily nursing care and services following the current federal, state and local standards, guidelines and regulations that govern long term care and assisted living facilities. You are entrusted to ensure that the highest degree of quality care can be provided to our residents at all times.
PRINCIPLE DUTIES AND RESPONSIBILITIES
- Receive and report resident status at the beginning and end of each shift and as needed.
- Provide personal care to residents per their individualized plan of care.
- Document all pertinent information regarding care as assigned.
- Report all changes in residents’ conditions to the Nursing Supervisor as soon as practical.
- Prepare residents for meals and assist to and from the dining areas.
- Record residents’ fluid/food intake.
- Assist residents with toilet, dental, hair, and bath care.
- Keep residents dry and clean.
- Make room checks and verify resident location.
- Keep resident’s rooms neat and clean.
- Assist residents with dressing/undressing as necessary.
- Prepare residents for activity and social events (church, services, parties, visitors, etc.)
- Transport residents to/from appointments, activity and social programs, etc.
- Assist residents to walk with or without self-help devices as instructed.
- Perform vital signs, B/Ps, weights and other trained procedures as directed by management.
- Complete prevention and restorative care that includes but not limited to PROM, AROM, ambulation, transferring, bathing, grooming, and all other ADL care.
- Assist in turning, positioning, and repositioning of residents.
- Attend care plan meetings as assigned.
- Demonstrate a passion for caring as evidenced by interaction with co-workers, residents, families, and visitors.
- Perform all other duties, as assigned.
EDUCATION/EXPERIENCE/JOB TRAINING
- Must possess, as a minimum, a 10th grade education.
Experience
- Must be a licensed Certified Nursing Assistant in accordance with the laws of this state.
Specific Requirements
- Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
- Must be able to read, write, speak, and understand the English language.
- Must posses the ability to deal tactfully when personnel, residents, family members, visitors, government agencies/personnel and the general public.
- Must have patience, tact, a cheerful disposition and enthusiasm, as well as willingness to handle difficult residents.
- Must not pose a direct threat to the heath or safety of other individuals in the workplace.
