CHC Nurse Assessor North East
Band 5 and Band 6 Continuing Healthcare Nurses (RGN) (RMN) (RNLD) (RSCN) are needed in the North East area.
Job Summary
The post holder will support the Continuing Healthcare Specialist Nurse/Clinical Lead and the wider team to ensure that the workstream is planned and managed effectively and ensure their successful delivery:
• To be responsible for managing a defined caseload of Complex Cases
• To ensure Continuing Healthcare guidance is followed and that services are delivered to maximize patient safety by meeting their ongoing clinical needs
• To participate in ongoing professional development and clinical supervision for themselves and for other nurses within the team.
• To co-ordinate and facilitate Multi Disciplinary assessments and patient reviews
• To co-ordinate and undertake timely reviews for patients who have care packages/placements in place
• To participate with other team members to ensure that the commissioned services provide the necessary care needs to minimise risk to the patient.
Essential criteria
• Professional Qualification in nursing
• Clear communicator with excellent writing, report writing skills; capable of constructing and delivering clear ideas and concepts concisely and accurately for diverse audiences
• Ability to analyse and interpret information, pre-empt and evaluate issues, and recommend and appropriate course of action to address the issues
• Able to prioritise and plan workloads, working on own initiative and organise own workload without supervision working to tight and often changing timescales
• Must have access to a car and hold business insurance as the post holder may be required to work at any establishment at any time throughout the duration of their contract (3 months plus, with potential to go permanent 37.5 hours per week, Monday-Friday)
Desirable criteria
• First line management qualification or experience or evidence of working at this level for at least 2 years
• Experience in communications and stakeholder management
• Previous experience in similar role in public sector
• Experience of working effectively with clinicians, social care and other partner agencies.
