Clinical Care Coordinator- Daly City
Come join our exciting Hospitalist Program Seton Medical Center located in Daly City, California. Cogent Healthcare is seeking a Clinical Care Coordinator (CCC). The CCC is a registered nurse who collaborates with the hospitalist care team to coordinate and manage the process of patient care from the point of admission through discharge and transfer of care to the primary care provider. A successful candidate will be energetic, reliable, and have a strong sense of ownership.
Essential Functions
Clinical Care Coordination
- Actively participates in daily standup meeting with hospitalist team to ensure communication and implementation of patient plan of care.
- Collaborates with hospital care team and hospital staff to facilitate the timely implementation of patient care plans, especially at transition points along the continuum of care.
- Meets with patient/family to set expectations for hospital stay, verify primary care provider and determine potential barriers to discharge.
- Collaborates with hospital discharge planning to ensure correct patient status (i.e. observation, inpatient), appropriate and safe discharge plan and disposition.
- Communicates pertinent information regarding patient’s hospital stay (landmark events) with patient’s primary care provider as needed.
- Obtains information relevant to the patient’s hospital stay from their primary care provider.
- Ensures appropriate post-discharge follow-up appointment is arranged with primary care provider.
- Resolves post-discharge homecoming call issues.
- Support and application of established processes for insurance verification.
Quality Improvement
- Participates in data collection for quality improvement initiatives: PQRI, review of core measure indicators, and collects care-delay information.
- May participate in client and care team meetings to evaluate and plan progress of the hospitalist program.
- Work on QEP; from data gathering, being part of the process improvement project, reminding the physicians regarding appropriate documentation.
Education/Public Relations
- Meets with patient/family on admission to educate on role of hospitalist.
- Meets with patient/family to provide education on disease process and expected LOS in the hospital
- Meets with patient/family to provide education on medications and possible adverse effects.
- Continued follow-up with patient/family to provide follow-up PCP appointment and assure needs are being addressed during stay.
- Educates and promotes hospitalist program among hospital staff.
Schedule:
- Typically Mon-Friday (possibly 1 weekend per month, with the option to work from home.)
- Start time is 7am
- Position is full time and salaried
- Some flexibility may be required depending on the business needs.
Education, Training, Experience
- Graduation from an accredited school of nursing; BSN
- Licensed to practice as a Registered Nurse (RN) in state of CA
- Adult acute care experience - Required
- Case management experience – strongly preferred
- Utilization management experience (preferred)
- Well-developed communication and influence skills
