Job Description : Clinical Case Manager The Clinical Case Manager works collaboratively with interdisciplinary teams and healthcare team members both internal and external to the organization to improve patient care through effective utilization and monitoring of healthcare resources and assumes a leadership role to achieve desired clinical, and financial outcomes. The Clinical Case Manager coordinates care and services of selected patient populations through the acute care episode and across the continuum. As it relates to resource management, the Clinical Case Manager views the organization as an interrelated system of processes and links case management initiatives to organizational goals and objectives. The Clinical Case Manager utilizes standards, guidelines, and protocols for care delivery and incorporates data to continuously improve care and outcomes. The Clinical Case Manager ensures improvement in practice settings by assuming responsibility for life long learning and demonstrates leadership skills in decision making and problem solving.
