Major Job Functions:
- Responsible for organization and supervision of the administrative functions of the hospice program.
- Responsible for the management of the hospice program in matters of overall operation at the local office level.
- Assure that Care Team meetings are conducted and records maintained.
- Assures that care is in accordance with Medicare, Medicaid, Federal and State guidelines.
- Responsible for employing qualified personnel and ensuring employee orientation, training education and annual employee performance evaluations as per SouthernCare policy.
- Assures complete orientation of all clinical staff as per SouthernCare policy.
- Conducts employee counseling, resignation and termination paperwork. Submits to Corporate HR as per SouthernCare’s policy procedure.
- Assure appropriate review and follow-up for all revocations to minimize occurrence.
- Follow-up on any patient, employee, customer complaints/concerns. Report to RCD with action plan.
- Assure discharge planning process as per SouthernCare policy and procedure.
- Meet/contact SNF DON’s on a regular basis to evaluate satisfaction with hospice services.
- Oversees and assures compliance with Performance Improvement Program and assures updated policies, forms, job descriptions are reviewed with appropriate staff and utilized in the office.
- Participates with RCD in interview and hiring process for LMD.
- Conducts and records regular staff meetings and participates in weekly CRD/CD staff meetings.
- Participates in weekly CD conference calls with RCD, completes the weekly report and faxes it to RCD. Maintains file for follow-up.
- Completes Monthly Variance Report
- Participates in Regional CD meetings.
- Attends Annual Memorial Service.
- Assumes direct patient/family care when needed.
- Assures all direct care clinical staff are assigned appropriately.
- Participates in On-Call, carry caseload as staffing/census levels require.
- Assures CTC is provided when indicated.
- Assures that reports are received on new admissions during normal business hours.
- Participates in and assures that medical record review is conducted as per SouthernCare policy.
- Assures that evaluation visits are conducted on all potential discharges.
- Conducts employee observed visits per SouthernCare policy.
- Monitors and approves bills from DME vendors and from EHO; forward to corporate office.
- Arranging and negotiating services provider through contracted agreement.
- Reviews and follow-up on RCD chart audit findings.
Requirements:
- Is a full time employee within the geographic area of service.
- Registered nurse with current State Employment license. BSN preferred.
- Two year supervisory experience preferred.
- Must have good knowledge of hospice Medicare, Medicaid, State and Federal guidelines.
- Demonstrated ability to supervise and motivate.
- Must be able to maintain patient confidentiality at all times and be able to abide by HIPAA rules and regulations
- Must be proficient with computers, and hand held technical devices such as Smart Phones
