The Clinical Team Lead will be the collaborator between Evercare and the physician group who is providing care to over 150 members in nursing homes in Albuquerque. This position will also include some clinical care. Responsibilities of this position include:
- Education of mid-level providers regarding the Evercare model and components including quality, documentation, and other components of the Evercare model.
- The establishment and maintenance of effective relationships with the Evercare members, key decision-makers, facility staff and contracted NPs.
- Performing chart and quality audits to determine compliance with contract elements.
- Ensuring the capture of essential data into electronic medical records.
- Attending regular monthly meetings with contract PCP group.
- Supervision of RN Liaison.
- Interacting with members, families, physicians, and key nursing facility staff.
- Serves as a clinical expert.

Requirements
- Current, unrestricted NP license the State where this position is located (or reciprocity)
- Certification as a GNP, ANP, CNS, or FNP preferred
- DEA license required
- Prior management/supervisory experience preferred.
- Prior long term NP experience preferred.
- Proficiency with computers and electronic medical records, including intermediate level of proficiency with MS Office.
- Strong management and leadership skills, with knowledge of long-term care, geriatrics and chronic illness
- Proven experience in developing and sustaining internal and external client relationships with healthcare professionals. Active in the medical community.
- This position requires a self starter who is adaptable to a changing environment and health care reform.
