Lead Nurse Practitioner
The Evercare Clinical Team Leader is responsible for managing approximately 25-50% case load and direct supervisory responsibility for 2 to 6 Evercare Mid-Level practitioners. The Supervisor, Nurse Practitioner reports to the Manager, Nurse Practitioner. Position has potential to convert to full time manager position.
Responsibilities:
- Candidate should be able to travel throughout Monmouth and Mercer Counties to service our nursing facilites.
- Providing direct primary care to aging residents in long-term care facilities in collaboration with primary care physicians
- Coordinating care by collaborating with primary care physicians, consulting physicians, facility staff and patients' families
- Participating in care conferences with other providers and patient family members
- Educating nursing home staff, physicians and administration.
- The Clinical Team Leader will also serve as the direct supervisor for a team of 2 - 6 Nurse Practitioners
- Provides primary case management of chronic and acute conditions. - Reviews the work of others.
- Develops innovative approaches.

- Serves as a leader/ mentor.
- Coordinates, supervises and is accountable for the daily activities of business support, technical or production team or unit.
- Impact of work is most often at the team level.

- Positions in this function require license and certification as Nurse Practitioner, Physician Assistant, or Clinical Nurse Specialist or ability to obtain
- If NP, certified nurse practitioner through ANCC/AANP as adult, family, and/or geriatrics (GNP preferred) or ability to obtain
- If PA, masters prepared or Physicians assistant in compliance with state or ability to obtain
- DEA certificate or ability to obtain
- Successful experience within a managed care organization as PA/NP in a LTC setting strongly preferred
