Job Description:
VillageCare's Certified Home Health Agency (CHHA) is seeking experienced professional Community Health Nurse's (NYS RN) for Fee-For-Service home visits in the Bronx, Brooklyn, and Manhattan.
The CHN provides professional nursing care and services to assigned patients in their homes according to Physician's orders, Nursing Standards of Practice and the agency's policies, procedures and protocols. The CHN is also responsible for coordinating, implementing and evaluating the delivery of client services under the direction of the Branch Manager and the CHHA Administrator.
Fee-For-Service positions available for weekdays, weekends, and holidays.
Requirements:
Minimum Requirements: Current NYS RN license and registration, with an Associate's Degree in Nursing, and one (1) year professional job related experience as an RN in home care setting.
A Bachelor's Degree in Nursing is preferred.
