CTT Nurse - Adult CTT
Western Psychiatric Institute and Clinic is currently seeking a CTT (Community Treatment Team) Nurse for the Adult CTT. Under the direction of the CTT Program Coordinator, the CTT Nurse provides comprehensive mental health services to residents of Allegheny County in community settings, including but not limited to, homes, hospital emergency rooms, personal care homes, etc. The CTT Nurse is responsible for demonstrating clinical competence in the delivery of direct patient care and the implementation of the milieu management program. The primary responsibility is to provide intensive outpatient mental health treatment in a variety of settings to consumers with serious and persistent mental illness for whom traditional outpatient mental health services have not been effective. Responsibilities -Serves as primary case coordinator for a caseload of consumers. -Provides quality care to consumers following the Assertive Community Treatment (ACT) Model of mental health care delivery. -Acts as an advocate for the consumer and/or family and provides advocacy for consumers, including 302 and other involuntary commitment proceedings, guardianship, and custody proceedings. -Participates in outreach visits to consumers in various housing/living situations. -Collaborates with team members to provide needed services to coordinate with other community agencies. -Provides consultation and direct assistance to the DEC or community ER staff in the case of at risk consumers presenting for treatment; this includes diversion of consumers from a psychiatric ER in accordance with ACT Standards of Care. -Maintains accuracy and completeness of patient records in compliance with institutional standards. -Participates in and/or completes a comprehensive assessment of identified clients. This includes social history, psychiatric history, and mental status exams. -Participates in team meetings in a collaborative manner. -Submits documentation that is professional in content and appearance by established deadlines. -Assists consumers to obtain and maintain culturally appropriate basic living needs through skills such as: food, medical care, recreation, education, and employment, through referral and assertive follow-up, advocacy, and problem resolution. -Assist consumer to set goals and plan to meet those goals based on consumer strengths, needs, interests and cultural preferences. -May facilitate or co-lead small groups of consumers/or significant others to increase understanding of consumer issues and to improve coping skills and performance of activities of daily living. -Participates in 24 hour on call rotation. -Participates in weekend rotation. -Acts as a liaison between CTT psychiatrists, local pharmacies, and local community health providers. -Monitors consumer pharmacotherapy adherence and response under the supervision of CTT psychiatrist. -Provides competent nursing care as determined by patient needs and nurse knowledge of patho physiology. -Provides psycho education as needed to consumers and family members to provide education regarding mental illness and its treatments. -Attends agency and state trainings as appropriate. Identifies training needs and ensures presentation of appropriate training necessary. Basic Qualifications -Graduation from an approved School of Nursing. -Minimum of two years of mental health experience required. -Current nursing licensure in Pennsylvania. -Experience in Clinical Psychiatry preferred. -The incumbent must by physically capable of providing direct total physical care to unpredictable, potentially violent, consumers as necessary. -Demonstrated knowledge of mental health laws required. -Ability to work collaboratively within a multidisciplinary team required. -Ability to work under stress, with an unpredictable consumer flow required. -Availability for a flexible schedule and rotating shifts required. -Must have access to reliable transportation. Licensure/Certifications -Current RN in Pennsylvania required. -Act 33/34/73 clearances required within 30 days of hire. -Valid drivers license and access to reliable transportation required.
