We currently have an exciting opportunity for a full-time Director of Coordinated Patient Services at our corporate office in San Jose, CA.
The Director of Coordinated Patient Services (DCPS) is responsible for developing strategies for and providing education to all stakeholders who participate in providing patient services to SHI patients to facilitate optimum individualized patient care. These activities include developing strategies to maximize early patient CKD education, to facilitate increasing utilization of home therapies, to mitigate factors influencing increased morbidity and mortality during the transition from CKD to ESRD, and in conjunction with Medical Affairs, to participate in implementation of programs to achieve clinical initiatives as defined by the CMO. This position requires effective communication skills both internally and externally. This is a high-visibility position requiring developing and maintaining strong relationships with individual physicians, corporate employees, center employees, and other stakeholders. The position participates collaboratively with the Regional Directors of Operations of both Satellite Dialysis and WellBound to implement programs and achieve goals.
The effectiveness of the position is measured by the number and percentage of patients participating in options and wellness classes, the percentage of patients choosing a home therapy, the percentage of patients enrolled in Optimal Start, and other annual clinical targets as defined by the CMO.
Required Experience:
5 years of nephrology experience and 3 years healthcare management required

RN preferred
Ability to travel
Experience with adult education preferred
Ability to function independently with minimal direction

Ability to produce clear organized project definitions which include clearly defined measurable goals
