Early Intervention Service Coordinator Job in Brooklyn 11226, New York US
The Early Intervention Program is a statewide program that provides various types of educational and therapeutic services to infants and toddlers with disabilities and their families. Early Intervention Service Coordinators are a central member of the multidisciplinary team and is responsible to assist families with all the steps to getting EI services for their child and ensuring that those services are provided as authorized.
Some of their responsibilities and duties are;
- Assess child/family concerns, priorities and resources,
- Develop and implement IFSP at initial meeting
- Complete paperwork for re-authorization of services every 6 months
- Monitor service provision
- Coordinate services between therapists and family
- Attend trainings and meetings as necessary
- Make home visits
- Bill for all service coordination activities
Bilingual Spanish, Haitian Creole and knowledge of case management and child development a plus
