Job Description
Penn Presbyterian Medical Center is currently seeking a full-time Clinical Resource Coordinator to provide case management and discharge planning services for the Emergency Department. Responsibilities will include interacting, supporting and consulting with physicians, social workers, and other disciplines to coordinate care. Duties also include evaluating severity of illness and intensity of service, as well as assessing patients' post-hospital needs.
Qualifications:
· Pennsylvania licensed RN
· Minimum of four years clinical experience
· BSN preferred
· Case Management or Discharge Planning experience preferred.
· Must be flexible with schedule and able to work 4-10 hour shifts which will include working every other weekend.
