Position Summary

 

The Family Liaison is responsible for identifying areas of need within a child’s ecology and arranging to remedy those needs beyond the program environment during enrollment as well as after discharge.  The Family Liaison is responsible for assessment, treatment planning and family work associated with the youth on their case load.  The Family Liaison also coordinates and implements all case management activities for assigned youth.

 

Essential Roles and Responsibilities


1.
         Treatment Planning

To assure comprehensive, continuous identification and implementation of treatment and service needs related to the child’s ecology for post-program success the Family Liaison (1) focuses treatment and services on  life domains including; family, education, social/recreational, financial, legal, medical and mental health as appropriate (2) uses procedures and strategies approved by the program to assess  the needs of the child, the child’s family, home, and community  (3) develops and facilitates a treatment team that includes members from each life domain (4) develops strength based plans based on the individual needs of the child and family (5) starts discharge planning at entry (6) provides and documents all case management functions (7) assures that all services provided to children and families are culturally competent and culturally sensitive .

 

2.         Family/Ecological Work

To keep youth connected to family and community and  improve youth and family functioning in  the community the Family Liaison (1)  arranges and coordinates visits between the youth and his/her family or other identified persons (2) regularly visits  each family’s home or other place of residence and makes continuing personal contact through home visits and phone calls (3)  leads the treatment team in identifying family and community resources/supports to assist the youth in returning to the community (4) provides family members with parenting skills as needed (5) participates in clinical supervision and implements interventions discussed.

 

3.         Communication

To assure continued communication between the program, family members, treatment team members and external agencies/supports, the Family Liaison (1) arranges for each youth’s family and treatment team members to participate in activities at the program (2) maintains regular contact with the youth’s placing agent and attends external meetings and court (3) communicates all program philosophies, goals and procedures to treatment team members (4) fosters and maintains collaborative relationships and communicates regularly with all professionals both within and outside of the organization (5) includes and informs all significant members of the child’s treatment team in important decisions and meetings  (6) shares information with teammates in the areas of the youth’s ecological assessment, current home and community developments, family interventions and individual conferences with youth .

 

4.         Supervision and Safety

To assure the continuous safety of all youth, the Family Liaison  (1) directly supervises assigned youth via visual and auditory contact as agreed upon with other staff (2) follows program-designated procedures for all physical contact with youth during physical restraints, youth searches and so on (3) follows program-designated procedures for youth illness and injury, informs appropriate persons of the situation, and documents contact (4) assists and supports other staff, as needed, in crisis interventions to protect youth, staff, and property (5) detects and remedies, with Director approval if necessary, any potentially dangerous environmental conditions (6)  maintains safe driving procedures when transporting youth (7) follows mandated reporting laws/expectations.

 

5.         Documentation and Record Keeping

To assure documentation is completed as required by program policies, procedures, licensing and funding sources, the Family Liaison (1) maintains youth’s permanent record (2) follows expectations for documentation of case management activities (3) follows licensing regulations regarding documentation (4) prepares monthly written reports as required (5) documents the development of various plans and Discharge Summaries (6) completes written work that is legible, accurate and uses professional language (7) meets regulatory and programmatic  timelines for submitting paperwork.    

 

 6.        Professional Development and Training

To assure consistent and continuous professional development and training,  the Family Liaison will (1) complete all training requirements as identified (2) participate in ongoing professional development in-service trainings (3) participate in clinical supervision (4) complete the minimum number of annual professional development training hours in accordance with regulatory guidelines (5) achieve professional development goals as identified in performance evaluations.
