Provision of Nursing Care services and coordinating all aspects of quality client care, with the aim to support the independance, dignity and safety of clients in their own home. The Community Nurse works within a multi disciplinary framework with individuals, families and the community. These services are delivered in a centre based clinic or the clients homes. Service provision includes technical care, early intervention, professional assessment, monitoring/maintenance, self management programs, education and health promotion.
