Home Care Occupational Therapist Job
Constellation Health Services – formerly known as Norwalk Rehabilitation Services/Health and Home Care Resources – provides a diverse yet interconnected range of health care services to the communities of Connecticut’s Fairfield and New Haven counties, and beyond. Our team of dedicated occupational and physical therapists, nurses, home health aides, and administrative staff are the collective “stars” that align to improve the well-being of all of the communities we serve.
Constellation is currently in need of per diem Home Care Occupational Therapists for the lower Fairfield County and Hamden/New Haven areas. Our occupational therapists work with patients to improve the abilities needed to perform self-care tasks such as grooming, bathing, dressing and other day-to-day activities associated with living safely and independently at home. We are looking for dynamic OT’s to help our patients achieve their goals.
