- The Registered Nurse Case Manger is responsible and accountable for the delivery of skilled, high quality patient care through use of the nursing process and in accordance with established policies and procedures of the department.
- He/she acts as a coordinator between the patient/family, physician and department services to provide for individualized planned and appropriate care specific to the needs of the patient. He/she demonstrates a high degree of technical, organizational and critical thinking skills, initiative, integrity, accuracy and nursing judgment.
- Additional functions applicable when acting as Charge Nurse: Makes patient care assignments based on the skill, experience, and temperament of other Home care personnel and on the acuity of the patients, ensuring patient/staff safety.
- Responsible to direct and precept staff members as needed/assigned in all aspects of critical care management.
- Participates in quality and process improvement in the department, which may involve planning, implementing and sustaining efforts.

- Graduate of an accredited school of Nursing, Bachelor's Degree preferred.
- Current CPR card, and IV qualified
- One-year home health case management experience preferred.
- Currently licensed as Registered Nurse in the state of Florida
