Home Infusion - Team Lead - Continuing Care Services

The Home Infusion Team Leader, under the direction and in partnership with the Home Infusion Manager provides clinical leadership for the home infusion team to enhance the delivery of timely high quality and cost-effective care which meets all regulatory, accreditation, licensure and certification requirements. Essential Functions: - The Home Infusion Team Leader will assist in the supervision of home infusion nursing personnel by: - Oversight of day-to-day staffing patient schedules - Assists w/ monitoring of staff performance, provides input into staff evaluations documentation of performance problems to Home Infusion Manager - Makes home visits w/ Home Infusion Nurses to assure employees providing home infusion care have knowledge skills required to provide age appropriate care to patients families through education, training or experience - Assists w/ interviewing process of applicants provide hiring recommendations to Home Infusion Manager - Leads case conference/team meetings - Provides direct nursing care manages the care of patients from admission to discharge, coordinating the multidisciplinary plan of care; including appropriate referrals to other disciplines; community resources; supervision of the home health aide plan of care; coordination of the discharge plan documentation to reflect all activities provided in program appropriate care settings - Assess the learning needs of the patient/caregiver to develop the plan of care that meets patient family needs goals including the provision of effective teaching, supervision delegation of care to achieve desired health care outcomes in the appropriate setting timely discharge consistent w/ quality care - Identifies clinical best practices facilitates their implementation - Participates in program/department quality management improvement activities: including but not limited to clinical record review, occurrence reporting, focused studies, process outcome measurement, peer review, continuous quality improvement projects, partners in defining achieving agency productivity standards - Provides expert practice consultation for the home infusion team, CCS outpatient skilled nursing facility staff - Assures professional competency through maintaining state of the art skills knowledge consistent w/ Medicare JCAHO regulations, applicable nurse practice acts agency policies - Maintains infusion nurse certification - Participates in home infusion call duty consistent w/ agency policy - Performs other duties as requested Qualifications: Basic Qualifications: - Two year medical-surgical nursing experience in an acute care setting - Minimum two years recent experience as a home infusion nurse or equivalent experience - One year leadership experience or commensurate experience - Baccalaureate degree in nursing or commensurate knowledge experience - Licensed in Oregon Washington. Requires Oregon or Washington license at time of hire. Licenses in both states are required w/in six (6) months of hire. - Valid driver's license - Current Basic Life Support CPR card - Thorough knowledge of principles of teaching, learning delegation - Working knowledge of interdisciplinary care coordination - Working knowledge of concise, yet thorough clinical documentation of patient assessments care - Demonstrates customer-focused service skills - Able to perform complete physical, psychosocial, spiritual assessments - Demonstrated ability to apply effective communication interpersonal relationship skills - Able to operate a motor vehicle in all weather conditions - Ability to use a laptop computer keyboard - Ability to lift 25-30 lbs - Ability to manipulate small objects involved in patient care (catheters, needles, point of care testing equipment, etc.) - Demonstrated ability to apply effective problem solving, communication interpersonal relationship skills, time management skills required to accomplish case management responsibilities w/in allocated time frames Preferred Qualifications: - Three years of medical/surgical nursing experience. - Medicare regulations. - Peripheral central line IV management. - Pediatrics, including well child care. - Oncology/hospice. - Rehabilitation nursing. - Hospice care the dying process. - PICC /midline placement certified. - Able to perform venipuncture IV insertion. - Able to operate multiple IV pumps. - CRNI certification. - Home health nursing. - Pediatric well child nursing. - Oncology hospice nursing. - IV therapy nursing. - Rehabilitation nursing. Salary Range: $34.10 - $48.32
