Home Visit Physician – INSPIRIS

The Physician is responsible for the delivery of health care services to a designated group of INSPIRIS patients in their homes. The Physician works collaboratively with other team members to provide improved access and high quality care through proactive, preventative care, early recognition and aggressive treatment in place when safe to do so, and proactive and ongoing advance care planning. These clinical programs include CarePlus, Transitions, and/or Assessments. The Physician reports to a Network Clinical Director or a Regional Medical Director. Delivers cost-effective, quality care to assigned INSPIRIS patients in their homes.
- Identifies high risk patients and prioritizes care appropriately
- Provides prognosis appropriate preventative care to INSPIRIS patients
- Identifies acute conditions proactively; develops and implements a plan of care and treatment goals that are consistent with the patient's wishes
- Manages chronic conditions effectively while remaining compliant
- Educates patients, caregivers, and others and engages them in increased self-management when possible
- Communicates effectively and professionally with all team members
- Performs comprehensive assessment and documents findings in a concise/comprehensive manner
- Manages behavioral issues alone or in collaboration with specialty provider
- Addresses Advance Planning with all patients in a proactive manner; identifies legal Medical Power of Attorney (MPOA), health care proxy, or legally authorized representative (LAR) when patient lacks decision making capacity
- Maintains communication with MPOA, health care proxy, or LAR on a regular basis per INSPIRIS standards
- Manages medications to effectively treat the patient and avoid inappropriate or unnecessary medications and meet health care forumulary
- Facilitates the implementation of the plan of care by engaging other team members (facility staff, nurse case managers, community providers, hospice team members,health plan case managers, etc)
- Evaluates the effectiveness, necessity, and efficiency of the plan of care and make revisions as needed
- Utilizes practice guidelines and protocols established by INSPIRIS
- Documents in accordance with INSPIRIS standards
- Enters / updates patient information in INSPIRIS Enterprise Systems (IES)
- Promotes and engages in INSPIRIS� customer service initiatives
- Handles complaints or concerns promptly
- Serves as an ambassador for INSPIRIS to our customers and to the community
- Participates in required meetings
- Provides collaboration with and oversight of assigned nurse practitioners or physician assistants as required by specific state regulations
- Meets program and market specific job responsibilities as assigned
- Achievement of targeted process, utilization, and quality metrics
- Adherence to documentation standards
- Medical record management per INSPIRIS standards
- Decreased involuntary turnover of patients
- Active, unencumbered medical license in State of New Mexico (M.D. or D.O.) required
- ABMS Board certification in Geriatrics, Internal Medicine, or Family Medicine required
- Geriatric experience preferred
- Chronic disease management experience preferred
- Managed care experience highly desirable
- Positive attitude and strong work ethic
- Self-starter with ability to practice with a high degree of independence
- Competent clinician who practices within scope of practice
- Superior reasoning, planning, organizational, and time management skills
- Strong verbal and written communication skills
- Proficiency in or ability to learn necessary computer skills
- Ability to meet deadlines; accomplish work in order of priority; maintain composure and effectiveness under pressure and changing business conditions
- Ability to adjust to new situations within a reasonable amount of time
- Ability to identify opportunities for improvement and develop solutions or innovations to improve outcomes
- Ability to work effectively and relate well with superiors, peers, and individuals / departments / entities inside and outside the company
- Ability to meet attendance requirements
