The SouthernCare hospice aide/CNA exhibits a caring attitude towards our hospice patients, and their families with maturity and the ability to cope with emotional and/or stressful situations. They assist with hands-on patient care in accordance with the Care Plan, and assists in maintaining a safe and clean environment under the direction and supervision of a Registered Nurse.
Essential Functions:
- Drives to patient’s residence to provide care; may visit several locations in a day
- Assists in maintaining a clean and safe living environment—such as changing bed linens, washing laundry; light cleaning of patient’s living quarters or arranging furniture to allow room for
- ambulation/transfer, and assuring that rugs are secured, equipment (such as wheelchairs) maintained in operational order
- Assists patients in and out of bed, automobile, or wheelchair to bathroom and up/down stairs
- Assists patients ambulating with assistive devices including canes, crutches, walkers, braces and artificial limbs
- Assists patients in activities of daily living such as feeding, bathing, dressing, grooming, shaving, and other related tasks
- Assists in maintaining patient’s nutrition—prepares and serves food following special prescribed diets
- Assists in taking vital signs (temperature, blood pressure, heart rate, respiration); collect and maintain records of fluid intake and output; record/report condition of patients
- Directs patients in simple prescribed exercises; assists in range of motion exercises/activities
- Understands and accommodates when it is important to take time to entertain the patient by reading aloud, playing cards and other games to provide companionship
- Performs variety of miscellaneous duties as requested, such as obtaining household supplies and running errands
- Maintains records of services performed and of apparent condition of patient reporting significant changes in the patient’s status to the Registered Nurse and participates in Care Team meetings
Other Functions:
- Participates in in-service education programs to maintain 12 hours of in-service annually
- Participates in weekend and holiday rotations as assigned by Supervisor
Requirements:
- Hospice Aide and/or Certified Nurse Assistant certificate in state of employment or home health training certificate required. For the State of Kansas, both certifications are required.
- Must be able to read, write and speak clearly and legibly
- Successful completion of SouthernCare competency evaluation.
- Must possess and maintain reliable transportation, current auto insurance and valid driver’s license throughout employment.
- Meet critical and emotional demands of Hospice Aide position.
- Must be able to meet the physical demands of hospice duties such as, but not limited to, lifting, transferring patients, carrying, pushing and/or pulling, stooping and bending on a frequent or daily basis
- Must be able to maintain patient confidentiality at all times and be able to abide by HIPAA rules and regulations
- Must be proficient with computers, and hand held technical devices such as Smart Phones
