Hospice Bereavement Counselor

As the Hospice Bereavement Counselor you will be responsible for bereavement visits, bereavement groups 2-3 time a week (There is one at 12 Noon on Thursdays and then another one held in the office at 6PM on Thursday evenings). You will have document point sign in sheets for groups. Your time will be equally spent in the office (mailings, phone calls, etc) as well as out in field (in patients homes if need be).
Responsibilities:
- Provide coordinated patient care in an interdisciplinary hospice setting
- Asses the patient/survivors emotional status of the patient and/or family.
- Develop and implement a patient goal oriented plan of care as a member of the clinical IDG
- Assist and implement a survivor goal oriented plan of care for bereavement issues.
- Develop, maintain, and contact as necessary, a resource group of clergy to whom specific aspects of spiritual care may be delegated.

Qualifications:
- Minimum one year of experience in the area of grief and loss and support group activities.
- Active licensed in the state of Nevada for Social Work, MFT, Certification in Bereavement or related field and/or equivalent and relevant education and work experience.
- Undergraduate degree or equivalent experience.
- Experience in program development and management.
- Experience in working with patients/families dealing with life threatening illness and death.
- Good computer skills-able to input data for bereavement program.
- Nevada drivers license and maintain personal auto insurance coverage.
- Must possess motor vehicle in good working condition that is available during working hours for job related tasks.
