Positions in this function require various clinical licensure/certification based on role. May include: RN, LPN/LVN, CHHA. Non-RNs work under direct supervision of someone with RN or MD licensure as part of an interdisciplinary team. Function is responsible for overseeing and participating in day to day clinical care of terminal patients and their families. Function is also responsible for clinical care and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating) as it relates to end of life palliative care.
Primary Responsibilities:
- CHHAs work under direct supervision of someone with RN or MD licensure as part of an interdisciplinary team
- Participates with Hospice team members in the development and implementation of the patient's and family's plan of care
- Provides support for and reinforces the team's teaching of the patient's caregivers
- Communicates with Hospice RN case manager regarding services provided and significant findings and changes in patient condition
- Applies knowledge/skills to activities that often vary from day to day
- Performing physical activities that require considerable use of your arms and legs and moving your whole body, such as climbing, lifting, balancing, walking, stooping, and handling of biohazard materials
- Requires current unrestricted CHHA licensure in applicable state.
- Must be able to demonstrate customer service excellence and communication in a team setting.
Requirements:
- High school diploma/GED is required.
- Must be a certified CHHA in California.
- Basic computer proficiency required
- Must have a valid driver's license, reliable transportation and insurance
- Ability to give status update to RN on patient care required
- 1+ years of experience as a CHHA preferred
