Hospice Medical Social Worker
Provide wide range of psychosocial services to terminally ill patients and their families, including regularly assessing the patient and family psychosocial needs, and Pre-Bereavement/anticipatory grief needs. Implements the Plan of Care provisions to meet their needs, which include: social service goals for alleviating problems, supportive counseling, problem solving, community referrals pre-bereavement and bereavement care. Regularly re-evaluates the patient's and family's psychosocial needs. Adheres to Company policies and procedures and regulatory compliance requirements and directives.

Requirements

* Minimum age requirement of 18.

* Bachelor's degree in Social Work, Master's degree preferred. Masters degree from school accredited by the Council on Social Work Education required in states as specified.

* Licensure and/or certification as required by the State.

* Minimum of two year's social service experience in a health care setting or social services agency.

* Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, etc.).

* Compliance with accepted professional standards and practices.

* Understands Hospice philosophy, principles of death/dying.

* Ability to work in an interdisciplinary setting.

* Physical exam in accordance with state law requirements.

* Valid driver's license and an automobile that is insured in accordance with state and/or Odyssey requirements and is in good working order.

* Satisfactory references from employers and/or professional peers.

* Satisfactory criminal background check.

* Compliance with accepted professional standards and practices.

* Ability to work within an interdisciplinary setting.

* Demonstrate excellent observation, judgment and communication skills. Ability to provide good written documentation in a timely manner.

* Self-directed with the ability to work with little supervision.
