Key Support Worker Job Description Bradford Kirklees
Location: Base centre to be either BEST Bradford, Huddersfield or Batley centres (in 2012 our Batley centre will be re-located to Dewsbury)
Responsible to: Contract Manager
Job Purpose: To manage a caseload of individuals and provide them with a comprehensive package of support on both an outreach and centre based structure. A car and full driving license is an essential requirement for this role.
Responsibilities:
1. To manage individual cases of single or multiple family members moving individuals through a range of support activities and progress measures to move them closer to, and into the labour market.
2. To provide barrier removal activities to family and individual issues and provide a package of personalised support that engages participants with the benefits of employment.
3. To work within a community and family setting developing both individual and family lead action plans.
4. To move a minimum of 22% of participants into work through a comprehensive package of support.
5. To provide a range of general training requirements to individuals and deliver and/or refer into accredited and non accredited learning opportunities.
6. To tackle personal and family barriers through holistic and pastoral support and the delivery of a range of modular sessions that will make up the programmes “Progress Measures”.
7. To provide appropriate referrals to a range of support agencies that can add value for participants without duplicating existing support.
8. To work in partnership with Support Workers and Managers of Statutory and Non Statutory Agencies to develop a joint, multi agency approach to tackling each family’s specific barriers.
9. To participate in Multi Agency Case Reviews where possible/appropriate with other statutory and non statutory agencies who are currently supporting family members.
10. To provide feedback to the Local Authority through agreed and approved mechanisms.
11. To achieve targets for individuals to achieve Progress Measures and Job Outcomes as agreed with the Contract Manager
12. To proactively engage with the BEST Appraisal and Performance Management system.
13. To continuously improve the customer experience.
14. Comply with the company Equality and Diversity, Health and Safety and Security policies.
15. To carry out any additional duties in consonance with the role as required by the Contract Manager.

Experience

Qualifications (Any of the attached would be desirable):
Q1 NVQ Level 3 Information, Advice and Guidance qualification is desirable
Q2 PTTLS/CTTLS/DTTLS are desirable
Q3 NVQ Level 3 in Health Social Care qualification is desirable

A1 A full driving Licence is essential
A2 Use of a vehicle to travel across all sites is essential
