At APS Healthcare, our mission, quite simply, is to improve the health of those we serve. We are excited to be working with the state of Tennessee to provide a full continuum of health management and wellness services to the State’s employees, including lifestyle management, disease management (DM) and case management services. The new ParTNers for Health program will assist members with their health care decisions, including providing a nurse advice line, as well as biometric screenings and preventative services. The program goal is to deliver greater value to enrollees, providers, and the State of Tennessee, through improving access to appropriate health care for the program enrollees.

We invite you to help us make your community healthier and better by joining this new and exciting program! We have an opportunity available for a Licensed Clinical Social Worker. This telephonic position provides telephonic disease management and/or case management services, focusing on member engagement, education and empowerment to self manage their chronic health conditions. The Social Worker supports and encourages healthy lifestyle choices, health promotion, and behavioral change through motivational interviewing and incorporating behavioral change theories to reduce long term effects of chronic disease.

Responsibilities:
•Provides telephonic health-coaching and consultation services to participants, within state licensure scope of practice requirements. Conducts health risk assessments, introduces case management services, identifies and addresses barriers to treatment adherence and educates members about healthy lifestyle choices within state licensure scope of practice requirements.
•Collaborates with participants to establish goals and provides ongoing support to members they strive to make and sustain healthy lifestyle choices. Provides participants with educational material and program introductory material, providing reinforcement of educational topics.
•Collaborates with primary care/treating physicians as well as medical home and other healthcare professionals to obtain specific current health information in regards to members’ plan of care.
•Reviews data from variety of sources to assist in identifying co-morbid conditions, hospitalizations, prescriptions and other factors that could impact the educational and support of the individual. Integrates data identify problems, set goals, plan interventions and determine overall health status within scope of practice.
•Collects clinical data and enters information into care management system for tracking and metrics purposes.
•Monitors and evaluates the services and community based resources necessary to respond to the individual health needs.

Education:
•BA/BS in Social Work, Behavioral health, or related field.
•Master’s Degree in Social Work required.
•Licensed Professional Social Worker (LCSW) required.

Qualifications:
•Minimum 3 years of medical social worker experience.
•Knowledge of adult learning and behavior change principles preferred.
•Knowledge of community based resources.
•Strong clinical assessment skills, consistent with state licensure scope of practice requirements to support long term behavioral change.
•Ability to demonstrate purposeful and outcomes directed critical thinking within daily operational activities of care management delivery using sound judgment and facts in carrying out disease management activities.
•Ability to integrate data and evaluate to design meaningful interventions.
•Strong telephonic interviewing and assessment skills.
•Strong computer skills, e.g. care management applications, Internet/Web, Microsoft Office (Word, Excel, Outlook).
•Strong prioritization and organizational skills.
•Excellent written verbal communication skills.
