Position Description:

POSITION SUMMARY:
Manages a caseload of SMI consumers and provides case management services. Works collaboratively with the CM team to implement a client centered individualized recovery plan (IRP). Completes the Case Management Assessments, to include the acuity determination and the Individual Recovery Plan. Locates and secures community-based services according to client’s desires and resources available. Conducts intensive outpatient (IOP) mental health treatment in accordance with best practices to seriously mentally ill members.

ESSENTIAL FUNCTIONS:
1. Manages a caseload and oversees the coordination of all CM services to include the development of an Individualized Recovery Plan (IRP), completion of the case management assessment, acuity determination, crisis plan reviews and utilization review (as needed). (30%)

2. Conduct IOP sessions as designed and prescribed as part of the individuals’ treatment plan in groups which average approximately 10 members. (30%)

3. Documents encounters and submits paperwork supporting these activities in a timely manner (10%).

4. Collaborates with the Case Management Team to review case management process for SMI Consumers meets with Care Coordinators to gather/provide feedback to ensure needs of members is met. (15%)

• Makes collaborative outreach attempts, including transportation of consumers as needed, to other involved or needed providers to coordinate member’s care as necessary. (5%)

• Maintains referral network with other human service agencies in the community and makes the appropriate referrals to other programs, community organizations, etc. (5%)

• Participates in the on-call rotation. (5%)

EDUCATION:
• Current Hawaii Licensed Social Worker (LSW)/Licensed Clinical Social Worker (LCSW) ; or Licensed Marriage and Family Therapist (LMFT); or Licensed Mental Health Counselor (LMHC); or Licensed Advanced Practice Registered Nurse (APRN); or
• Hawaii Licensed Registered Nurse with two (2) years of clinical practice experience in the behavioral health field.
• Requires Bachelor’s Degree in a Behavioral Health related field. Masters Degree in a Behavioral Health field preferred.

EXPERIENCE:
1. Minimum of two (2) years experience in direct inpatient and/or outpatient mental health clinical care preferably in behavioral health field.
2. Group facilitation experience preferred.

KNOWLEDGE, SKILLS ABILITIES:
1. Knowledge of psychosocial rehabilitation and recovery values, approaches and interventions
2. Knowledge of various public and private services available for the mentally ill and the substance abuser.
3. Knowledgeable in case management software systems and Microsoft Office suite of application is desirable.
4. Ability to provide clinical assessment, crisis intervention, and therapeutic skills with mental health and/or substance use disorders
5. Ability to effectively manage groups of individuals to ensure a successful and safe learning environment.
6. Ability to teach groups of individuals of mixed cognitive levels and psychiatric states a variety of information topics and skills
7. Ability to ensure appropriate clinical care is provided.
