New Horizons is now recruiting for a Licensed Mental Health Practitioner-Substance Abuse Specialist/Asst. Team Leader.

General Summary:

Functions as a member of FACT, a self-contained Clinical Team, which provides needed treatment, rehabilitation and support services to identified "members" with severe persistent mental illness.  This member may bring, or be assigned to develop secialty areas of expertise such as substance abuse treatment, vocational rehabilitation, housing development, or benefits coordination.

Provides case management for an assigned group of members includign coordinating and monitoring the activities of the individual treatment team (ITT); assume primary job responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plans in conjunction with the ITT; provide individual supportive therapy and symptom management; ensure immediate changes are made in treatment plans as member's needs change, educate and support members' families and advocate for members' rights and preferences.

Provides on-going assessment of members' mental illness symptoms and members' response to treatment; substance abuse screening and assessment of co-occurring disorders.

Provides individual vocational-supportive counseling to enable members to identify vocational strengths and problems, establish vocational or career goals and plans to reach them, and recognize and target symptoms of mental illness that interfere with work.

Provides on-going assessment, problem solving, side-by-side services, skill training, supervision to assist members with activities of daily living. 

Assists the Team Leader in the day-to-day administrative operations and supervision of the FACT team.

Qualifications:  Must be licensed in the State of Florida as a mental health counselor, clinical social worker, marriage and family therapist.  Must have a degree in a behavioral science and work experience with adults with severe and persistent mental ilnesses and have a strong focus on treating co-occurring disorders.  Must have a strong commitment to the right and ability of each person with a severe and persistent mental illness to live in normal community residences; work in market jobs; and have access to helpful, adequate, competent and continuous supports and services.  Must have a valid Florida Driver's license with 6 points or less in the last 3 years.
