Job Description Licensed Practical Nurse: The Licensed Practical Nurse is a licensed nurse who participates in the plan of care to provide competent, compassionate and individualized nursing care to patients and their families-as developed by a registered nurse or nurse practitioner. The LPN also participates in the medical plan of care under the supervision of the physician or nurse practitioner and in activities to facilitate safe and efficient practice operations. REPORTS DIRECTLY TO: Nurse Coordinator and Admin Manager GENERAL RESPONSIBILITIES: 1.0 Provides leadership and mentoring role to medical assistant staff 1.1 Participates in orientation of new permanent and temporary clinical staff members (e.g. medical assistants, LPN, RN) 1.2 Performs teaching of other clinical support staff for whom she/he is responsible 1.3 Appropriately utilizes MGH nursing resources 2.0 Participates in the plan of nursing care as developed by, and under the supervision of, a registered nurse or nurse practitioner 2.1 Provides and maintains a therapeutic environment for patients and families 2.2 Communicates the plan of care and other pertinent information to/from other team members 2.3 Provides nursing care appropriate to age group of patients consistent with the practice scope of service 2.4 Observes, reports and documents pertinent patient information, in accordance with the standards of documentation for ambulatory care nursing 2.5 Performs nursing procedures as outlined in the Nursing Procedure Manual 2.6 Assists the registered nurse or nurse practitioner to evaluate outcomes of nursing care for patients and to modify the plan of care as appropriate 2.7 Works with the registered nurse or nurse practitioner to define standards of excellence for patient care 2.8 Participates in and contributes to performance improvement activities 3.0 Participates in the patient intake process with medical assistants 3.1 Assists with the patient intake process: ¿ Escorts patient to exam room and checks identification ¿ Obtains information about the chief complaint ¿ Reviews current medications and prescriptions needing refill ¿ Obtain vital signs ¿ Documents all of the above in LMR 3.2 Instructs/assists patient to prepare for examination by MD including setting up special equipment as needed 3.3 Notifies provider when patient is ready and communicates pertinent information obtained during the intake process 3.4 Assists patient through check-out process when exam is complete 3.5 Cleans and prepares exam rooms 3.6 Responds to needs to expedite visit and keep on schedule 4.0 Participates in the medical plan of care under the direction of the physician or nurse practitioner 4.1 Assists provider with patient examination, procedures and treatments as directed 4.2 Administers medications including vaccines 4.3 Collects and tests specimens. Instructs patients about specimen collection at home as applicable 4.4 Performs phlebotomy 4.5 Performs other tests, procedures and treatments as directed. Documents in LMR per practice protocols ¿ Vital Signs ¿ Visual Acuity ¿ Portable 02/cannula setup ¿ Nebulizer Treatments ¿ Dressing Changes ¿ Spirometry Testing ¿ Mantoux Tests ¿ Pap Smear/Vaginal Cultures ¿ Fecal Occult Blood ¿ Ear Irrigation ¿ Ear Drop Instillation ¿ Pulse Oximetry ¿ Electrocardiograms 4.6 Performs designated tasks to track test results, via LMR, and communicate results to patients 4.7 Receives patient messages, via Clinical Messaging, reviews with provider and takes action as directed 4.8 Faxes, mails or telephones in prescription renewals as directed by providers and practices Medication Auto Renewal policy 4.9 Assists provider in coordinating care with/making referrals to specialist, testing, etc. 5.0 Collaborates with other professional and non-professional personnel in maintaining recognized standards 6.0 Participates in orientation of new permanent and temporary clinical staff members (e.g. medical assistants, LPN, RN) 6.1 Performs teaching of other clinical support staff for whom she/he is responsible 6.2 Appropriately utilizes MGH nursing resources 7.0 Participates in daily activities conducive to the comfort and safety of patients, their families and practice staff 7.1 Appropriately utilizes equipment and supplies 7.2 Provides input to RN and Administrative Coordinator regarding the practice clinical needs 7.3 Participates in practice and other committees for the formulation of practice policies and procedures 7.4 Participates in monthly staff meetings 8.0 Professional Development: maintains and updates clinical knowledge and skills 8.1 Collaborates with RN or NP to identify and meet own learning needs 8.2 Participates in relevant continuing education programs 8.3 Adheres to regulatory requirements for annual training QUALIFICATIONS: ¿ Current licensure as licensed practical nurse in the Commonwealth of Massachusetts ¿ 1+ years of previous LPN/Nursing experience ¿ Excellent organizational and customer service skills ¿ Associate?s Degree or similar higher education LIMITATIONS: ¿ Does not take verbal medical orders ¿ Does not take telephone medical orders WORKING CONDITIONS: ¿ Fast-paced, busy medical office environment ¿ General clinical exam room areas with laboratory ¿ Position will require candidate to be able to lift 10-100lbs.
