LPN Continuous Care – Extended Night Shifts
The Licensed Practical Nurse is responsible for coordinating the patient's and family's care. Administers and provides nursing care according to the plan of care to terminally ill patients admitted to the VITAS program, in accordance with the Nurse Practice Act in the state where the VITAS program is located. Teaches families and other primary care persons appropriate care techniques. Provides accurate documentation with visit itineraries. Provides direct skilled bedside nursing care. Evaluates patient /family needs in the preparation and executions of the interdisciplinary plan of care for each patient on the team. Evaluates effectiveness of treatments and monitors patients reactions. Maintains a clean and safe environment. Maintains accurate clinical notes on interdisciplinary charts. Reports significant findings or changes it the patients condition to the Registered nurse, Team Manager or Physician. Documents reports in clinical notes. Participates in Nurse LPN on- call rotation/schedule in order to meet the needs of patients and families.
Completion of course of study as required acquiring state licensure.
Qualified candidates must be currently licensed as a Licensed Practical Nurse. At least one year, preferable two years of satisfactory nursing experience. At least one year of which LPN in a hospital on an adult medical surgical unit.
