Major Functions:

 

The primary function of the Healthcare Coordinator is to provide coordination of medical care for clients.  The Healthcare Coordinator will provide services relating to the promotion of wellness for clients through educational lectures, individual counseling and special topic seminars. 

 

The Healthcare Coordinator is a member of the interdisciplinary clinical team at their assigned facility and participates in case conferences, treatment and discharge planning meetings.  The Healthcare Coordinator serves as a liaison between the agency’s clinical treatment team and the external primary medical care provider ensuring continuity of care for clients with medical conditions.

 

Specific Duties and Responsibilities:

 

·        Educate and encourage clients with medical conditions in adherence to medication regimens.

 

·        Provide group and individual health education to clients.

 

·        Identify and develop with Director of Healthcare Services a curriculum of health education specific health education topics, such as, smoking cessation, tuberculosis,

sexually transmitted diseases, hepatitis, family planning, pregnancy, asthma, and diabetes that are coincidental to disease management of persons served.

 

·        Provide gender-specific health education, such as female health education (e.g., anatomy, physiology and gynecological issues) and male health education (e.g., prostate screening).

 

·        Develop programs that provide information relative to general health concerns to promote well-being for identified medical needs of agency clients.

 

·        Develop, assess, obtain and disseminate health-related information for clients.

 

·        Maintain Standard Precautions and as needed educate clients and staff in Standard Precautions

·        Assume responsibility for the maintenance (security/safety of medication storage) and general appearance/hygiene of the medication administration unit/area.

 

·        Document all assessments, interventions, and responses to treatment in progress notes of the clinical record following agency procedures for timely and complete documentation.

 

·        Review all medication orders and follow agency procedures for order, renewal or discontinuation of prescribed medications.

 

·        Maintain, set-up and document on medication administration record (MAR) as per agency procedures.

 

·        Maintain a controlled substance log and medication destruction log in accordance with NYS DOH regulation and agency policy and procedure.

 

·        Maintain flow sheets for diabetic clients receiving insulin therapy.

 

·        Ensure the maintenance of accurate, complete, timely and high quality client records that comply with external regulatory standards and agency policy and procedure.

 

·        Complies with 42 CFR confidentiality and HIPAA privacy and security regulations.

 

·        Ensure client confidentiality and privacy by obtaining appropriate consents for release of information.

 

·        Use communication skills effectively with on-site medical providers, clinical staff, and clients to maintain a positive working relationship.

 

·        Prepare/submit reports to Director of Nursing and Medical Director.

 

·        Provides Smoking Cessation education for clients and provides nicotine patch distribution as per agency procedures.

 

·        Performs other duties as requested.

 

 

QUALIFICATIONS:

 

 

1.      Graduate of accredited school of nursing: Bachelor’s Degree preferred.

 

2.      Current New York State license: Licensed Practical Nurse or Registered Nurse.

REQUIREMENTS:

 

 

·        Minimum of three (3) years nursing experience, in residential or ambulatory health care or behavioral healthcare settings.

 

·        Basic keyboard skills and computer literacy (Window based environment knowledge preferred).

 

·        Certification as a CPR/First Aid trainer preferred.
